judging by the root ball- the pot seems to be too big, it could be root rot. What kind of plant is it?
0 gillningarThe soil does not look good at all, it’s all wet and mushy appearing. You should probably remove it from the soil, get some new soil with slightly more drainage and more organic material to aerate the roots, and replant it with that new soil mix in a more appropriate pot for the size. It looks like you’ve gone a few inches too large of a pot, you want to have 1-2 inches from the root ball to the sides when replanting
