I would guess watering issue too without seeing the rest of the plant and the soil.

Possible issues: Stem rot - Looks like rot at the base of the dark stem. Check for a soft mushy feel on the dark stem. Root Rot - I highly recommend checking for root rot. Look for mushy smelly roots and soaked soil around it. Poor Drainage - The soil looks compacted. It’s hard to tell from just a photograph. I would replace the soil and use a standard potting mix with extra perlite to allow good drainage and allow the soil to dry faster giving oxygen to the roots. Too Much Water - I recommend not following a strict watering schedule without verifying the soil actually needs water. Use a bamboo skewer like you are testing a cake is done baking. Look for no moisture on the skewer after pushing it to the bottom of the pot and leaving it in the soil about 5 minutes. Very wet soil will leave dark wet areas starting from the tip that touches the bottom of the pot. I recommend using a wooden skewer even with a moisture meter. Moisture meters are not 100% reliable. 🍀🪴
