Can anyone tell me what’s wrong with my plant? Maybe if the leaves keep dying and falling off I don’t know what to do.

Are the leaves crunchy when they fall off or more soggy? If they’re crunchy, I would say under watering. If they are soft then it is over watering

It’s a tradescantia/inch plant. It looks like it’s dying from the base of the stems. That usually indicates root rot from overwatering, pests in the soil or possibly insufficient lighting. I recommend checking the soil for pests or root rot. Change out the soil. Use well draining soil with perlite. Tradescantia rots easily if it sits too long in water. Remove the dead decaying roots, stems and leaves. You can propagate the remaining healthy stems. See the picture. Water after the soil has completely dried out. Tradescantia stores a lot of water in its stems. Ensure the container has sufficient drain holes. 🍀🪴
