Overwatering and fungus gnats larvae eating the roots probably. Remove it from the pot. Check for root rot and cut off all rot. Disinfect with full strength hydrogen peroxide. Replace the soil with a well draining mix. Replace the pot if it has no drain holes. Only water when the soil is dry and not by a Planta schedule. It’s important to check for moisture in the soil with a wooden skewer or similar tool to prevent overwatering.
