I’m a new plant parent. Can someone please tell me what I’m doing wrong? Should I put this back in soil?

    no but have you been changing the water? also root rot can happen but if its recent then it can be shock

      I transferred it from soil to water about a week ago. Should I change the water every week?

      yes you should to prevent root rot but if i can ask why did u transfer from soil to water?

    Not all plants do well in water. Also water roots are different than soil roots so the plant is probably transitioning between the two.
