Hey there! Does anyone know what this things is underneath my leaf? Is it a new leaf coming in? Or is it anything bad? Any help would be appreciated!

      Why would it come out of the side? Why not at the top of the orchid?

      The stem on the orchid is the central axis of the plant that leaves grow out of. A flower spike is the thin twiggy growth that develops buds and blooms. Phalaenopsis flower spikes like these come out of the side of the orchid stem, between the leaves, usually at least two leaves down from the top. Yours looks like it had trouble breaking through under the second leaf down and made its way up and out at the leaf joint. Flower spikes rarely come out of the top of your orchid, or what we call the crown. You really don’t want that, as it means the spike will damage the crown of the plant, making it impossible for new leaves to grow, and therefore, essentially ending the development of your orchid.
