She needs a repotting. The bark mix will definitely break down over time and start to becomes slightly acidic. I believe I’m seeing some slight root burn on the one root you showed a pic. That and the roots on top of the media that are in pretty bad shape are my first indication. Next, see how the top two leaves are smaller than the previous leaves? That’s another sign that your orchid is stressed. New leaves that come in from the crown should be at least as big if not bigger than the previous. It’s good to see that. There are no signs of dehydration in the leaves, but it’s inevitable that you’ll run into it down the line without giving this plant some fresh new media. I would highly recommend a container with more ventilation. Holes or slits are generally found in orchid specific plastic clear pots. These are great in order to keep an eye on the roots. You might want to go up one size. I have a feeling with fresh new media, you’re gonna see a lot more root growth. If an orchid bark mix seems to work well in your environment and with your watering habits, then I would stick with that.
