My Venus fly trap has been turning black and getting holes on the mouths of it. Can anyone please help me?

It could mean several things.  
1: you don’t have enough nutrients in the soil  
2: it is in a place too dark and gloomy for it  
3: not sure if this is 100% true but I’ve seen things about that happening and it’s just a natural process.

They do not like nutrient rich soil, it will burn their roots.
