May be your water. Are you using tap?

I am. I had no idea that the tap water could be affecting it!

Ivy can be way tougher to grow than you'd expect, as a houseplant. I have a tiny one named "it'll die" so I don't get too attached lol. What I've done with dying ivy is to cut off all the dead parts (including roots), see what pot size I needed for whatever remained alive, and pot in a light and well-draining mix that has some organic matter that's not too nutrient-rich, like fine bark rather than compost. I also put it directly in a window that got bright inderect light. It's bouncing back, slowly, and wants water relatively lightly but frequently.
