I woukd say he’s either dryy or he’s rotting a little bit, cacti aren’t my large knowledgeable topic, but those are my guesses, if it’s dry bottom water it , and if it’s rotting I’d say get  
New substrate and start new

African milk trees start dropping their leaves if they get too cold, and some of the discoloration is likely due to that as well. I would move it away from the window in the winter months, somewhere where it can get light at a distance, without the cold.
