I’m a new plant parent. Can someone please tell me what I’m doing wrong? Should I put this back in soil?

no but have you been changing the water? also root rot can happen but if its recent then it can be shock

I transferred it from soil to water about a week ago. Should I change the water every week?

yes you should to prevent root rot  
but if i can ask why did u transfer from soil to water?

It didn’t seem to be doing well in soil

Not all plants do well in water. Also water roots are different than soil roots so the plant is probably transitioning between the two.
